- Description: In a propensity-matched TriNetX cohort of 4,846 adults with obesity and documented asthma, one-year recorded exacerbations were 1.7% with GLP-1-based therapy versus 4.0% with non-GLP weight-management drugs (risk ratio 0.44), with the authors ruling out causal inference.

An active-comparator retrospective cohort drawn from 72 US healthcare organisations compared adults with BMI ≥30 and repeated asthma documentation starting GLP-1-based therapy against those starting non-GLP weight-management pharmacotherapy, with 2,423 patients per matched arm. Recorded asthma exacerbation over 365 days was 1.7% versus 4.0% (risk ratio 0.44), with lower systemic glucocorticoid exposure, emergency or critical-care use and acute respiratory failure. The authors note residual confounding, persistence uncertainty and exposure misclassification.

*Why it is interesting: Adds a respiratory outcome to the widening list of non-metabolic associations reported for GLP-1 drugs in routine-care databases, though from a design that cannot separate effect from selection.*
