The study examined injection-schedule adherence to long-acting cabotegravir plus rilpivirine in virologically suppressed adults with HIV in Spain. The nationwide RELATIVITY cohort combined retrospective and prospective data, counting injections given within ±7 days of target. Of 3280 evaluable participants, 70.4% received all injections on time, 28.9% received 90–99.9% on time and 0.7% under 90%. At least one late injection occurred in 38.2% and two or more in 13.3%; virological failure stayed infrequent. In adjusted models, centre volume, prior ART duration, foreign nationality, BMI and comorbidity burden had small-to-moderate associations with late injections. The design was observational and partly retrospective, so it shows associations rather than causes.

*Why it is interesting: Long-acting injections move adherence from patient pill-taking to clinic scheduling. In this large routine-care cohort, only 0.7% had suboptimal adherence, though single late doses were common.*
