- Description: Among 311,585 Japanese patients starting romosozumab between 2019 and 2023, 11.7% of men and 7.9% of women had a myocardial infarction or stroke in the prior 360 days, below the 8.9-17.6% among users of other osteoporosis drugs, and those with such a history discontinued more often.

A nationwide claims study covering almost the whole Japanese population identified 311,585 people initiating romosozumab for osteoporosis between March 2019 and March 2023 and measured documented myocardial infarction or stroke in the preceding 360 days. Recent cardiovascular history was present in 11.7% of men and 7.9% of women starting romosozumab, lower than among users of other anti-osteoporosis medications (16.4-17.6% in men, 8.9-11.5% in women) and declining across fiscal years. Discontinuation was more likely in patients with a cardiovascular history, particularly women.

*Why it is interesting: Sets the cardiovascular history of romosozumab starters against users of other osteoporosis drugs in a near-complete national claims population, in a setting where the drug carries a cardiovascular caution.*
