The ARS-PARTICLES cohort followed 20 757 adults with atrial fibrillation in north-eastern Poland for a median 5.6 years. Annual ozone, nitrogen dioxide and PM2.5 were modelled and assigned by residential municipality from the previous year. Fine-Gray models accounted for competing deaths and adjusted for comorbidities, temperature, area-level income, light at night and co-pollutants. Each interquartile ozone increase (4.03 ug/m3) was associated with a composite of ischaemic stroke, TIA or stroke death. Subdistribution hazard ratios were 1.27 (95% CI 1.02-1.59) for the composite, 1.14 for cardiovascular mortality and 1.38 for stroke. Exposure was assigned by municipality from the address at entry, and anticoagulation and smoking data were unavailable.

*Why it is interesting: Ozone is less studied than particulates, and here it is associated with stroke risk at low annual concentrations. The cohort is a high-risk clinical group rather than the general population.*
