This study examined accelerometer-measured activity and mortality in 2785 US adults with metabolic dysfunction-associated steatotic liver disease (MASLD). Participants came from NHANES 2003-2006; MASLD was defined by the Hepatic Steatosis Index. There were 584 deaths, including 198 cardiovascular deaths, over a median 14.7 years. Each 10-minute increment of moderate-to-vigorous activity gave a hazard ratio of 0.80 (95% CI 0.71-0.91) for all-cause death. For cardiovascular death the hazard ratio was 0.72 (0.53-0.99); total activity showed weaker associations and sedentary time none. Estimates were attenuated using the US Fatty Liver Index, and activity was measured once at baseline.

*Why it is interesting: Separates intensity from volume of activity in a group with raised cardiometabolic risk. The attenuation under an alternative steatosis definition shows how case definition moves the estimate.*
