# ORBI score marks larger infarcts in STEMI but predicts 10-year death worse than age

A DANAMI-3 sub-study classified 1,574 STEMI patients without cardiogenic shock as high (ORBI 8 or above, n=273) or low risk and linked them to the Danish Civil Registration System for ten-year mortality. High-risk patients had higher peak CRP (72 vs 11 mg/L), larger final infarcts (15.7% vs 10.1% of left ventricular mass) and 41% versus 20% ten-year mortality (adjusted hazard ratio 1.47, 95% CI 1.16-1.85). The excess was concentrated early, with an adjusted hazard ratio of 4.79 within 30 days and 1.27 after the first year, and the score discriminated ten-year death less well than age alone (AUC 0.67 vs 0.79).

*Why it is interesting: An acute risk score separates inflammation and infarct size sharply, yet beyond the first year the association with mortality was largely explained by age.*
