Linked electronic health record and claims data covered 4.95 million patients in 21 identifiable US health systems. 26,153 adults with a first outpatient Z59 code were matched to uncoded patients; an administrative code was the negative control. Coding ranged from 1% to 24% of patients across systems, varying widely even after adjustment for patient mix. Coded patients had 67% more emergency visits (95% CI 1.61-1.74) and 43% more admissions (1.38-1.49). The administrative code reproduced 46% of the emergency association and 70% of the admission association. At national planning prices, excess acute care was $2,516 per coded patient-year; the code added nothing to risk prediction. Hardship itself cannot be separated from the propensity to be documented.

*Why it is interesting: Z codes trigger referrals and adjust risk, yet a code with no hardship meaning reproduced 46% of the signal. Coding ranged from 1% to 24% of patients across systems.*
