An exosome biomarker with CA19-9 detected early-stage pancreatic cancer in 1,785 people

A prospective study of 1,785 individuals in four countries reports that the PANXEON exosome-based blood biomarker, combined with carbohydrate antigen 19-9, detects early-stage pancreatic ductal adenocarcinoma with high sensitivity.

The study tested a blood-based exosome biomarker panel, PANXEON, alone and combined with CA19-9, in a prospective cohort of 1,785 individuals recruited across four countries. The combination is reported to reach high sensitivity for stage I–II pancreatic ductal adenocarcinoma, the stage at which the disease is rarely found at present. Performance figures beyond sensitivity, including specificity in screening-relevant populations, are set out in the full paper.

Why it is interesting: Pancreatic cancer mortality has barely moved because almost nothing is caught early; a prospective multi-country test of an early-detection assay is the first thing that could change that arithmetic.
