GLP-1 agonists and atrial fibrillation in non-diabetic obesity with kidney disease

GLP-1 users with obesity and CKD had 1.63% one-year incident atrial fibrillation, against 2.62% in unexposed patients.

The study emulated a target trial in TriNetX US network records, 2021-2024. Non-diabetic adults with obesity and chronic kidney disease stages 3-5 were included. Semaglutide, liraglutide or tirzepatide users were matched to unexposed patients, 7,326 per cohort. One-year incident atrial fibrillation was 1.63% versus 2.62%, hazard ratio 0.655 (95% CI 0.508-0.844). Ventricular tachyarrhythmias (HR 0.556) and all-cause hospitalisation (HR 0.668) were also lower. Being retrospective, confounding by indication and unmeasured prescribing differences cannot be excluded.

Why it is interesting: It extends GLP-1 receptor agonist outcomes beyond diabetes to a cardiorenal group. Atrial fibrillation, ventricular arrhythmias and hospitalisation were all lower in treated patients.
